Athletic
Team Registration Information
The
Ipswich High School Athletic Department runs registration periods for fall,
winter and spring sports each year. Each
registration period will be held two to three weeks prior to the start of that
sports season. To register for one of
our athletic teams, you must bring each of the following with you to the
athletic office:

A Physical Exam given within the last 13-months.
·
A completed Medical History Form.
·
A Permission Slip signed and dated by the athlete and a parent
or guardian.
·
The User Fee Form signed and dated by the athlete and the
parent.
·
A User Fee Check made payable to I.H.S. Athletics.
The
Medical History Form, Permission Slip and User Fee Form are found at the
end of the Student-Athlete Handbook (see link below). Copies of this handbook can also be obtained
in the athletic office.

For
registration days print out the user fee form, permission and agreement form,
and medical history form pages. Be sure to fill out, sign and date
everything! Bring the completed
forms along with your physical and user fee check and you are all set!
