Athletic Team Registration Information
The Ipswich High School Athletic Department runs a registration period for fall, winter and spring sports each year. Each registration period will be held one to two weeks prior to the start of that sports season. To register for one of our athletic teams, you must bring each of the following items with you to the athletic office:
A copy of a Physical Exam which was given within the last 13-months. (no standard form- any form your physician uses is fine)
A completed Pre-Participation Questionnaire form. (located in the student-athlete handbook)
An Agreement Form signed and dated by the athlete and their parent or guardian. (located in the student-athlete handbook)
A User Fee Check made payable to I.H.S. Athletics. (explained on the agreement form in the student-athlete handbook)

Winter: Monday November 29, 2010 (Monday after Thanksgiving)
